Carl's goo transformation unexpectedly resurrects Donut as a sentient, talking cat with extraordinary charisma and a permanent buff granting bonus stats per level. She receives a legendary crown that places her in the royal line of succession for the Blood Sultanate on the ninth floor, forcing Carl to escort her through the dungeon as her designated bodyguard in a party now called The Royal Court of Princess Donut.

After Donut reforms from goo, she speaks to Carl and Mordecai, insisting on being called by her full title. When Carl checks her stats, he finds her strength and intelligence have increased significantly, and her charisma has jumped to 25. Mordecai explains she received an Enhanced Growth buff that grants +1 to Strength and Intelligence per level and +2 to Charisma per level, but locks her Constitution at 2 and Dexterity at 8. She learns the Magic Missile spell from a tome in her reward boxes. She then equips a legendary item: the Enchanted Crown of the Sepsis Whore, a purple tiara that grants +5 Intelligence and the Good First Impression skill, but also places her permanently in the royal line of succession for the Blood Sultanate on the ninth floor. The crown's description warns that royal members must slay the Sultan and all other family members before descending to the tenth floor. Mordecai explains the crown is fleeting and will turn to dust if removed, spawning elsewhere in the dungeon. Donut renames Carl's party to The Royal Court of Princess Donut and appoints Carl as her Royal Bodyguard, tasking him with escorting her to the eighteenth level. Mordecai notes that while Carl can leave the party, the challenge on the ninth floor will be significant.
