Katia reveals she has possessed a mysterious locked box from the beginning of floor six that contains the Orchid of Eileithyia's Grace, which offers three possible permanent boons. Two seals are already broken; the final seal requires completing an assassination quest through the Guild of Suffering. She discloses her plan to escape the Sepsis Whore rule and reassures Donut they will not fight each other.

Katia confesses to wearing a purple tiara given by Eva before her death, revealing her monologue mistake cost her an advantage. She then discloses the Engaged Lock Box of the Night Wyrm, a celestial prize from flooding Larracos, which she's carried since floor six. The box contains the Orchid of Eileithyia's Grace and requires breaking three seals to open. Two seals were completed by killing Eva and Imogen respectively. The three possible boons are: tripling base Constitution, becoming a Celestial Attendant on floor twelve, or becoming pregnant and being exported to the surface. Huanxin claims she will help ensure both Katia and Donut survive if Katia repairs Yemaya's shrine—which Carl already did. Mordecai warns the arrangement is dangerous, as Huanxin is a ruthless player in the Ascendency games seeking Yemaya as a retainer minion, and powerful sponsors could target Katia on floor twelve. Katia chooses the Celestial Attendant boon to avoid surface exile. The final seal requires an assassination quest through the Guild of Suffering, accessed via the Penis Parade strip club on the Desperado Club's top floor, managed by Damascus Steel. Carl receives a quest marked on desiccated skin that cannot be spoken aloud: kill Astrid, a level 125 Bloodlust Sprite and assistant manager of the Desperado Club, to complete Katia's box unlocking.
